1. SERVICE DESCRIPTION
Party A provides Brow Lamination services, which involve lifting, shaping, and nourishing eyebrow hairs using specialized products to create a fuller, more defined look. Results typically last 4-8 weeks, depending on individual hair type and after care.
2. CLIENT RESPONSIBILITIES & COMMITMENTS
Party B agrees to the following:
• thoroughly researched the service and voluntarily consents to the procedure.
•Has no serious skin conditions, chemical allergies, or medical conditions that may affect the treat ment outcome.
• Understands that results may vary based on individual hair and skin conditions, as well as personal aftercare. To follow the aftercare instructions provided to maintain the best results.
3. RISKS & LIABILITY WAIVER
Party B acknowledges and accepts that:
• The procedure may cause mild irritation, redness, itching, or, in rare cases, hair shedding.
• Results are not guaranteed as they depend on individual hair texture, skin type, and aftercare.
• If any adverse reactions occur, Party A is not liable for medical treatment or compensation.
• No refunds or reimbursements will be provided under any circumstances.
4. WAIVER OF CLAIMS & LEGAL ACTION
Party B agrees to waive any right to file complaints, claims, or legal actions against.
Party A regarding dissatisfaction with the results or any unexpected side effects.
Party B understands that by signing this agreement, they are legally bound to these terms and will not pursue any legal action against Party A concerning the BrowLamination service.
5. PHOTO & VIDEO CONSENT (OPTIONAL)
• I consent to have my photos/videos taken and used for marketing purposes.
• I do not consent.
6. FINAL AGREEMENT
By signing below, Party B confirms that they have read, understood, and agreed to all terms and conditions outlined in this agreement.
